Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), Ureaplasma parvum (UP), Mycoplasma genitalium (MG), Mycoplasma hominis (MH), Ureaplasma urealyticum (UU), Candida albicans (CA), Trichomonas vaginalis (TV), Gardnerella (GA), Herpes simplex virus type 1 (HSV-1), herpes simplex virus type 2 (HSV-2), Treponema pallidum (TP)

Common symptoms of genital tract infections
Abnormal vaginal discharge, vulvar itching, menstrual dripping, menstrual pain, lower abdominal distention, lumbosacral pain, dysuria, urgency, frequency, dyspareunia, bleeding during intercourse

Transmission routes and harms of genital tract infections
● Genital tract infections can spread unknowingly during sexual intercourse or pregnancy, often causing asymptomatic insidious infections:
The incubation period is long, the asymptomatic period is long, the course of the disease is long, the insidious is strong, it is difficult to cure, and the recurrence occurs
● Genital tract infections, if left untreated, can lead to long-term, irreversible, potentially fatal consequences:
Chronic pelvic pain, cancer, ectopic pregnancy, infertility, poor pregnancy prognosis, neonatal death, congenital malformations

Infection pathogens of the reproductive tract
More than 30 different bacteria, viruses and parasites are known to be transmitted through sexual contact, with eight of these pathogens associated with the greatest incidence of sexually transmitted diseases. In 2020, WHO estimated that 374 million people were newly infected.

One of the following 4 curable sexually transmitted infections exceeds 1 million infections per day.

● Chlamydia (129 million people)

● Gonorrhea (82 million people)

● Syphilis (7.1 million)

● Trichomoniasis (156 million people)

There is no cure for 4 pathogens. There are 400 million obstetrics and gynecology outpatient visits in China every year, of which about 50% are patients with reproductive tract infections.

● Hepatitis B virus

● Herpes simplex virus

● HIV

● Human papillomavirus

The preferred method of testing for genital tract infections is nucleic acid amplification test
● It is difficult to distinguish empirically because of the high co-infection rate of multiple pathogens and similar clinical symptoms.
● Traditional detection methods have low accuracy and low detection rate, which is prone to misdiagnosis and missed detection.
● The culture method has a long detection cycle and complex methods, which is difficult to apply in routine screening.
● UP and UU cannot be typed by culture method, and can only be typed by nucleic acid detection.

Nucleic acid amplification test is the preferred detection method for genital tract infection pathogens such as CT/NG/mycoplasma, and regular screening is recommended

“Diagnostic Criteria for Gonorrhea”, “Expert Consensus on the Diagnosis and Treatment of Mycoplasma genitalium Infection”, “Consensus on the Diagnosis and Treatment of Chlamydia trachomatis Infection in the Female Genital Tract”, “Guidelines for the Prevention and Treatment of STDs (2015 Edition)”, “WHO Standardized Diagnosis and Treatment Manual for Male Infertility”, etc

Recommendations for screening and diagnosis of reproductive tract infections
Guidelines at home and abroad suggest that attention should be paid to nucleic acid testing of reproductive tract infections and timely treatment according to the infection status.

●Guidelines for the Clinical Treatment of Sexually Transmitted Diseases in China, Centers for Disease Control and Prevention (CDC): Annual screening of sexually active women < 25 years of age, and screening of women at high risk of infection > 25 years of age (eg, with new sexual partners, multiple sexual partners, or sexual partners with genital tract infections).
● China’s Guidelines for Pre-Pregnancy and Pregnancy Health Care (2018 Edition): CT/NG is required to be used as a pre-pregnancy and early pregnancy preparation item to strengthen CT/NG screening in people of childbearing age and during pregnancy and childbirth, which will play a positive role in promoting the realization of eugenics and fertility and the prevention and control of CT/NG infection.
● China’s Expert Consensus on the Diagnosis and Treatment of Mycoplasma genitalium Infection: If both men and women have no symptoms related to genitourinary tract infection, and only Uu is positive, they are considered carriers and do not need treatment. Uu urethritis is present in men, and it is recommended to treat the partner at the same time. Patients with Uu detected in the lower genital tract during pregnancy do not require intervention and treatment, and both men and women are recommended to treat each other for a course of treatment when the semen quality is abnormal and there is a need for fertility. Positive genital Uu culture in both men and women had no significant effect on IVF.
●WHO Standardized Manual for Male Infertility: Mycoplasma males and chlamydia are common causes of epididymitis In the semen of infertile patients, herpes simplex virus and human papillomavirus (HPV) are highly detected, and HPV may affect sperm motility in semen.

Principles of multiplex nucleic acid detection for genital tract infection
The nucleic acid detection project of reproductive tract infection adopts real-time fluorescence PCR technology and hydrolysis probe technology to realize the qualitative detection of one or more pathogens at the nucleic acid level in the same reaction tube, which is simple to operate, and the whole process is not opened to reduce pollution, and the instrument software system automatically draws a real-time amplification curve to achieve instant result judgment. This test item is equipped with an internal standard, which is monitored during the collection, transportation and extraction of the sample to be tested through the internal standard to avoid false negative test results.

Advantages of the testing package
● Comprehensive detection: multiple target amplification in a single reaction tube, one sampling, one experiment, and simultaneous detection of 12 pathogens;
● Accurate classification: It is conducive to the detection of single infections and mixed infections, and accurately guides the effective treatment in clinical practice;
● Strict quality control: strict quality control standards, monitoring the whole process from sample collection to result interpretation to ensure the accuracy of test results;
● Open platform: short detection time, high throughput, open detection platform, compatible with a variety of fluorescence PCR instruments on the market.
